![]() * A typical completed report was rather thick, compared to older days when a 2-page questionnaire was used. * In the mid-80's when I was Indiana State Director I had additional tools of the time: a word processor, a copy machine and flat bed scanner. 1) When a witness was first interrogated, his/her story, without interruption, was recorded, the later transcribed into text. It included how the event began and where the witness(s) were. After the deposition I would use the questions I had jotted down during the recording. This was called the transcript. 2) The blank investigation sequence guide was completed as the the steps were taken and this was called the Investigation Activity Log. This was the report, the paper work. 4) Ray Fowler and I had devised additional forms. When an appropriate form was needed those were completed. Form 3 was for an E-M case. Form 4 was used for animal reactions. Form 5 was needed for medical data. Form 6 was used when there was physical evidence. Form 7 was for entities observed. Form 8, photographic or video evidence. Form 9, radar. Form 10, radiation. Form 11 was for sightings from aircraft. The Form 2 was a computer input form to help somebody with the data they needed to get the case properly inputted. MUFON changed these forms and the numbers are now different. Maps, diagrams, drawings, photos were included in the report. 5) Photos, audio and video tapes were sent with the report to MUFON HQ. Any important case I had investigated prior to the mid-80's was redone and updated, and submitted. This included NICAP cases I was directly involved in. Any report by any of my Indiana FI's was treated the same way. 6) Generated copies of each report were sent to 1) Original, MUFON HQ in Sequin, Texas 2) Copy to Dan Wright, Deputy Director of Investigations, Michigan. His evaluation form was attached to all copies before they were completed. When Wright retired he sent all his copies to MUFON. 3) Copy to Center for UFO Studies, Northfield, Illinois 4) Copy retained at my office. Later when I had to downsize and move to Newburgh, Indiana I sent all my copies to an archive in Michigan suggested by my colleague, Jean Waskiewicz. I no longer have copies of these detailed and signed reports. MUFON cannot find any of them so far and claim they are in the Pandora or MARRS files. CUFOS files may be harder to access but copies of the reports were sent there as well over 30 years ago. *** |
