Using Dan's information, I found the Blue Book references. NARA-PBB93-318 is the Archive's high contrast b/w rendition of the old photo. The Maxwell BB index lists the witness as "Miller."  The case number is 2583, July 1953. The case file itself is not yet on-line at Project Blue Book. For the record, the official Blue Book verdict was "hoax."
